Insurance Information
Insurance plan benefits, eligibility and billing addresses change frequently.  In order to ensure proper billing and to verify that the information stored within our system is correct, we require that you present your insurance card at each office visit. Checking your insurance card only takes a minute and it can ensure accurate and timely billing.  According to Hamilton Cardiology Associates policy, our office staff is directed to ask for this information and may be unable to proceed with your check-in without it. We greatly appreciate your cooperation with this process as it is our goal to make your check-in as smooth as possible.  If you have any concerns or comments with this policy, you can contact us through our patient portal.

Affordable Care Act
New health insurance exchanges, created by the Affordable Care Act (ACA), have recently become available to the public. Hamilton Cardiology Associates will be participating in the following HealthCare Exchange insurance plans:

Please be advised that Hamilton Cardiology Associates will NOT be participating in HORIZON BCBS OF NEW JERSEY’S HORIZON ADVANCE EPO product, but will continue to participate with all other BCBS Plans.

Please note this is not an inclusive list; many insurance companies contract with multiple insurance companies/products.  Even though a company is not listed, this does not mean that we are not participating.

Patients should contact their insurance company to confirm participation if not listed below.

If indeed your insurance plan requires a co-pay or referral, please be prepared to make such co-payment and/or present your referral at check-in.
