Insurance Information

Insurance plan benefits, eligibility and billing addresses change frequently.  In order to ensure proper billing and to verify that the information stored within our system is correct, we require that you present your insurance card at each office visit. Checking your insurance card only takes a minute and it can ensure accurate and timely billing.  According to Hamilton Cardiology Associates policy, our office staff is directed to ask for this information and may be unable to proceed with your check-in without it. We greatly appreciate your cooperation with this process as it is our goal to make your check-in as smooth as possible.  If you have any concerns or comments with this policy, you can contact us through our patient portal.

Please be advised that Hamilton Cardiology Associates will NOT be participating in HORIZON BCBS OF NEW JERSEY’S HORIZON ADVANCE EPO product, but will continue to participate with all other BCBS Plans.

Important Medicare Update: $2,000 Annual Cap on Part D Drugs Coming in 2025

Starting January 2025, Medicare beneficiaries will see a significant change: a new $2,000 annual cap on out-of-pocket expenses for covered prescription drugs. Once you reach this limit, you’ll receive catastrophic coverage for the remainder of the year, eliminating out-of-pocket costs for covered Part D medications.

Patients should contact their insurance company to confirm participation if not listed below.

If indeed your insurance plan requires a co-pay or referral, please be prepared to make such co-payment and/or present your referral at check-in.
